Primary testicular lymphoma (PTL) is a rare extranodal malignancy arising from the testis, accounting for approximately 5% of all testicular tumors and 1–2% of non-Hodgkin lymphomas, with diffuse large B-cell lymphoma being the predominant histological subtype. It is typically described as a disease of elderly males, whereas testicular tumors in younger individuals are more commonly non-seminomatous germ cell tumors, often associated with elevated tumor markers such as alpha-fetoprotein and beta-human chorionic gonadotropin. We report a case of a 40-year-old man who presented with painless, progressively enlarging bilateral testicular swelling over a period of 2–2.5 months. Clinical examination revealed enlarged, non-tender testes bilaterally. Ultrasonography demonstrated bilateral testicular enlargement with diffuse heterogeneous echotexture, particularly involving the right testis measuring 6 × 3.6 cm, suggestive of diffuse pathology. Notably, tumor markers were within normal limits, posing a diagnostic challenge. The patient underwent bilateral high inguinal orchiectomy, and histopathological evaluation confirmed non-Hodgkin lymphoma. PTL is an aggressive malignancy with limited data guiding its management due to its rarity; however, orchiectomy remains both diagnostic and therapeutic, and early initiation of systemic therapy is recommended. This case is noteworthy for its atypical presentation with bilateral synchronous involvement, occurrence in a relatively younger patient, and absence of tumor marker elevation, mimicking seminomatous pathology and contributing to diagnostic ambiguity.

Acute appendicitis continues to be one of the most prevalent surgical emergencies globally, presenting significant diagnostic challenges due to its diverse clinical manifestations. To standardize evaluation and enhance diagnostic accuracy, clinical scoring systems have been developed. Among the most commonly employed are the Alvarado score, the Pediatric Appendicitis Score (PAS), the Appendicitis Inflammatory Response (AIR) score, and the Adult Appendicitis Score (AAS). These instruments incorporate clinical signs, symptoms, and laboratory parameters to categorize patients based on the probability of appendicitis. Nevertheless, their diagnostic efficacy varies across different patient populations, including pediatric, adult, elderly, and pregnant cohorts. This review evaluates the validation and clinical applicability of various scoring systems, emphasizing their strengths and limitations across diverse patient groups. Overall, clinical scoring systems function as valuable adjuncts rather than standalone diagnostic tools. Their effectiveness is optimized when combined with clinical judgment and imaging techniques. Further refinement and population-specific validation are crucial to improving diagnostic precision and minimizing unnecessary surgical interventions.

We present the case of a 33-year-old patient who sustained a detonator explosion injury to the left hand, initially resulting in amputation of the last two fingers (little and ring fingers) as well as a large, complex wound. Primary management consisted of surgical debridement and lavage, empiric antibiotic therapy, and local wound care. After guided healing, a 6 × 3 cm skin defect persisted. Reconstruction using a MacGregor flap was performed. The article details the steps of marking along the groin crease, flap elevation, transfer to the hand, pedicle immobilization, and the second-stage division procedure. The outcome was favorable, with good flap integration and satisfactory functional recovery. The discussion reviews the indications, technical variations, and alternatives to this flap.

Acute appendicitis is one of the most common surgical emergencies worldwide, with laparoscopic appendectomy increasingly regarded as the gold standard treatment. Since its introduction in the late 20th century, laparoscopic surgery has transformed the management of appendicitis by offering reduced postoperative pain, shorter hospital stays, earlier return to normal activities, and improved cosmetic outcomes. This review examines the current evidence regarding the role of laparoscopic appendectomy in the surgical management of acute appendicitis in adults, children, and the elderly. Evidence suggests that laparoscopic appendectomy is superior in many clinical scenarios, although surgeon expertise and institutional resources remain critical determinants of outcomes.
